Registration of Research with Human Blood, Body Fluids or Tissues

Research Registration Form

Registration is required by the Biological Hazards Committee for all research with human blood, body fluids or tissue. This information is necessary in the Division of Environmental Health and Safety (EHS) for the identification of biohazard laboratories and the management of occupational exposures to biological hazards. Please fill out the attached form and return it to the Biological and Chemical Safety Officer at EHS.

As an integral part of this registration, principal investigators are responsible for:

  1. Filing the registration prior to the initiation of work and notifying EH&S when work with human body fluids is terminated or when changes in personnel or procedures occur.
  2. Establishing laboratory procedures for safe use of human body fluids based on guidelines provided by CDC/NIH. Work with human blood, body fluids, tissues and cells should be performed at Biosafety Level 2 (BSL 2) (See Biosafety in Microbiological and Biochemical Laboratories, HHS Publication No. CDC 84-8395.   3. Assuring that lab personnel receive biosafety training from EHS and any additional training that is necessary to carry out the job functions.
  4. Evaluating with occupational health physician and Biosafety Safety Officer for need of immunization and routine medical monitoring of lab employees.
  5. Making the appropriate protective supplies and equipment available to lab personnel.
  6. Informing personnel of procedures to follow in case of an accidental exposure to human blood or body fluids and the necessity to promptly report suspected exposure to the principal investigator.
  7. Reporting accidents or exposures to the Biological and Chemical Safety Officer at EH&S (545-2862)
